Program Review
Review the permit-required confined space program within one year of each entry using cancelled permits to determine if the program adequately protects employees.
Rescue Team Practice Drills
Each rescue team member must practice making permit space rescues at least once every 12 months using simulated rescue operations in actual or representative spaces.
Entry Training (Initial)

Attendant
Must know hazards including signs of exposure, monitor entrants, maintain an accurate count, remain outside the space, communicate with entrants, summon rescue when needed, and perform non-entry rescue when required.
Entry Supervisor
Must know hazards, verify entry conditions, authorize entry by signing the permit, terminate entry and cancel permits when conditions change, and ensure rescue services are available throughout the entry.
